ASSISTANCE SERVICES

The following forms are available to those who would like assistance with educational needs:

The Quartz Valley Indian Reservation’s Tribal Education Program gives Tribal members financial assistance in meeting their educational goals.  Applicants must be an enrolled member of the Quartz Valley Indian Reservation.  There are no financial guidelines and the scholarship is paid directly to the student as long as all criteria are met. Applications may be obtained from the Quartz Valley Indian Reservation Tribal Office or by clicking on the any of the forms listed below.

All Providers need to be enrolled in the Trustline Registry to be eligible to receive subsidy payment for Child Care through QVIR.  Please contact the Education Department to schedule an appointment through Trustline.

Providers registered with TrustLine send a strong signal to parents that they are committed to extending good care for their children. If no disqualifying criminal convictions or substantiated child abuse reports are found, your name will be added to the TrustLine registry.
